About Us

 

Bronx Community Health Network, Inc. (BCHN) was founded in 1996 as a community-based, not-for-profit organization and Federally Qualified Health Center (FQHC).

 

As an FQHC with funding from the Health Resources and Services Administration's Bureaus of Primary Health Care and HIV/AIDS, BCHN provides a comprehensive range of preventive, primary, diagnostic, pharmacy, specialty and related social support services to residents of medically underserved New York City communities, especially families and individuals without health insurance.  A “health center without walls”, BCHN contracts with Montefiore Medical Center and Promesa Systems, Inc., major Bronx health and human services providers, for the delivery of these health care services at seven community and three school health centers in the Bronx, New York. 

 

Community Health Promotion and Education

 

The goal of our community health promotion and education program is to encourage and motivate Bronx residents to make healthy lifestyle choices, to take charge and be responsible for their health and well-being. 

 

Our Patient Navigator Program recruits and trains community residents that represent a cross-section of the Bronx's multi-cultural and multi-ethnic population to inform Bronx residents about our programs and services, to provide linkages to other support services and to facilitate access to a personal primary care doctor, as well as assistance with public health insurance applications.

 

Through funding from the Yankee Foundation, we offer Dance For Fun, a youth fitness program featuring Zumba Latin rhythms, at local community and after school centers.   Seniors and adults participate in B'Bongo Fitness, aerobics, muscle-toning, stretching and relaxation exercises to African and American R&B rhythms.

 

 

On February 3, 2010, we launched a year-long Healthy Heart, Healthy Brain campaign to educate Bronx residents about the risk factors for heart disease and stroke, early recognition of signs and symptoms, how and where to seek early treatment and how to effectively manage heart disease and stroke if they have already been affected.  This initiative, which  aims to reach 100,000 Bronx households via a number of health promotion and social networking strategies, is funded by the American Heart Association/American Stroke Association and is supported by the Office of Bronx Borough President, Ruben Diaz, Jr., BronxNet and a number of Bronx-based health and social service organizations.

 

Governance

 

BCHN’s 14-member Board of Directors is comprised of a majority of members who are patients of the health centers, as well as individuals that either live in the Bronx, or have an interest in the health and well-being of Bronx residents.  Board members bring a wealth of experience and expertise to BCHN from a number of professions including community and union organizing, finance, education, social work, legal and health care.  Each health center also has an Advisory Board that consists of the health center’s clients.  In addition to their governance and fiduciary responsibilities, members advocate for the needs of health center patients.

 

The management team carries out the day-to-day work of BCHN in collaboration and partnership with its contractors and a number of Bronx-based, New York City, state, and national organizations.
