About Us:
Bronx Community Health Network, Inc. (BCHN) was founded in 1996 as a community-based, not-for-profit organization and Federally Qualified Health Center (FQHC).
| As an FQHC with funding from the Health Resources and Services Administration Bureau of Primary Health Care, BCHN provides a comprehensive range of preventive, primary, diagnostic, pharmacy, specialty and related social support services to residents of medically underserved New York City communities, especially families and individuals without health insurance. |

A “health center without walls”, BCHN contracts with Montefiore Medical Center and Promesa, Inc., major Bronx health and human services providers, for the delivery of these health care services at five community and two school health centers in the Bronx, New York.
BCHN’s 16-member Board of Directors is comprised of a majority of members who are clients of the health centers, as well as individuals that either live or in the Bronx, or have an interest in the health and well-being of Bronx residents. Board members bring a wealth of experience and expertise to BCHN from a number of professions including community and union organizing, finance, education, social work, legal and health care. Each health center also has an Advisory Board that consists of the health center’s clients. In addition to their governance and fiduciary responsibilities, members advocate for the needs of health center patients.
The management team carries out the day-to-day work of BCHN in collaboration with its contractors and a number oflocal, state, and national organizations.
