Executive Team

Eleanor Larrier, MPA - Chief Executive Officer
When Eleanor Larrier joined the fledgling Bronx Community Health Network (BCHN) in December 1999, she became the organization's first full-time executive director. BCHN has since grown from five to 18 community and school-based health centers, providing 109,000 Bronx residents with comprehensive, affordable quality health care in partnership with Montefiore Health System and Acacia Network/Promesa Systems. In keeping with the strong legacy of health center pioneers, Ms. Larrier has developed an innovative Community Health Promotion and Education Center of Excellence that recognizes the importance of complementing and integrating community assets with health center clinical care for the best possible patient and community health outcomes. Through a team of patient navigators, community health workers, emergency department liaisons and health educators that reflect the Bronx's diverse populations, BCHN arms Bronx residents with information to prevent and manage chronic illnesses and to have a voice in how health care is delivered in their communities.

Through Ms. Larrier's leadership, network health centers have implemented evidence-based programs including integrated, co-located mental/behavioral and primary care services, care coordination through integration of community health workers into multidisciplinary patient care teams and emergency room diversion, connecting patients with primary care doctors.

As a skilled planner, administrator and team builder, Ms. Larrier has progressive healthcare experience ranging from organizational development of community based organizations, program development and facilities planning for acute care hospitals and ambulatory care centers, and administration of hospital acute and ambulatory care services. She served as Associate Executive Director for the Departments of Planning, Clinical Inpatient Care Services and Ambulatory Care at Harlem Hospital Center, New York City Health and Hospitals Corporation. Prior to that, she was Associate Executive Director for Planning and Program Development at Elmhurst Hospital Center.

Ms. Larrier has an MPA in Health Policy, Planning and Administration from New York University, Wagner School of Public Service. For several years she served as an adjunct lecturer in health care administration at Lehman College, City University of New York (CUNY) and as member of the Advisory Committee for Lehman College's Master of Public Health Program.

Sharon Edwards-Bandyopadhyay, MBA - Chief Financial Officer
Sharon Edwards-Bandyopadhyay has served as Bronx Community Health Network's (BCHN) CFO for the past 20 years. She holds a Master's Degree in Business Administration(MBA) from Long Island University and has over 24 years of hospital and financial accounting, grants management and financial audit experience. Ms. Edwards-Bandyopadhyay provides support to the CEO, the Board of Director's Audit & Finance Committee, and supervises the financial management staff in the development of monthly financial reporting, budgets and Federal reporting submissions.

Prior to joining BCHN Ms. Edwards-Bandyopadhyay worked in Montefiore's Grants Management and Contracts Division, Montefiore Comprehensive Family Care Center - Assistant Administrator and Memorial Sloan Kettering Cancer Center.

Serving the community has always been her personal mission. She has been instrumental in acquiring grants seniors, directing summer programs for underprivileged youths in the South Bronx, fundraising for Bronx Presbyterian Churches scholarships fund and participating in missions for Haiti.

Jay Izes, MD - Chief Medical Officer/Quality Improvement Director
Jay Izes, MD joined became the Chief Medical Officer of BCHN in 2009. He also serves as BCHN's Quality Improvement Director and Compliance Officer.

Dr. Izes received his undergraduate degree from the University of Pennsylvania and his medical degree from Temple University. He is trained in Pediatrics at Montefiore Medical Center where he was a chief resident. He served as a major in the United States Army Medical Corps for two years and then entered private practice in Yonkers for more than twenty years including serving as Director of Pediatrics at St. Johns Riverside Hospital in Yonkers.

Leaving private practice in 2000, Dr. Izes served as Medical Director for outpatient service at Yonkers General Hospital and Valentine Lane Family Practice. In 2001 he was appointed Vice President for Medical Affairs and Chief Medical Officer at St. Johns Riverdale Hospital.

Tashi Chodon, BSN, MPH - Director of Programs
Tashi Chodon, BSN, MPH has over 20 years of experience with various government and non-government organizations in public health program management and design, technical assistance, training, and education. Her work has included programs in smoking cessation, child health, HIV/AIDS prevention, epilepsy, asthma, nutrition physical activity, diabetes and health care access. Ms. Chodon is especially qualified in the sensitive adaptation of health programs to cultural minorities.

In her current position, Ms. Chodon is the Director of Programs for the Bronx Community Health Network (BCHN). Ms. Chodon provides management oversight for the development of high quality, cost effective and integrated programs within BCHN and with contractors and community-based partners, in collaboration with the BCHN executive team. In her last position, Ms. Chodon was the Director of the Epilepsy Foundation's National Center for Project Access (NCPA), a clearing house for data collection and program evaluation for nine state demonstration projects funded by the US Health Resources and Services Administration (HRSA). The goal of Project Access is improving access to services in currently under-served communities.

Welcome to Bronx Community Health Network, Inc. (BCHN) and to our new and improved website. It is filled with information to help you stay healthy, and we welcome your comments.

We want you to think of your BCHN health center as your medical home. What do we mean by medical home? It's a health center where you are always welcome; where you will not be refused care if you don't have health insurance or the money for your visit to the doctor. At your BCHN medical home, our health care team will take care of your needs and treat you with respect.

Health center services are low cost or free based on your household income. Our outstanding team of doctors, nurses and patient care technicians and other health care professionals are ready to give you the best quality care possible. We put you, our patients, at the center of the health care team. We know that when we listen to you and get you involved in your plan of care, you are more likely to take control of your health. We want to motivate you to be your best self, to reduce your risk of illness, get fit and stay healthy!

BCHN's 17 community and school-based health centers are owned and operated by Montefiore Medical Center and the Acacia Network/Promesa Systems, Inc, two Bronx institutions that are known for providing high quality medical and substance abuse care.

BCHN is a unique organization. As a Federally Funded Qualified Health Center, more than half of the members of our Board of Directors are also patients at our health centers. They receive the same quality care and services that you do. As patients and Directors, they help us maintain high standards of care. They let us know what works and what doesn't and help to find the best ways to meet your needs.

There are many reasons to make your BCHN health center your medical home. They number one reason is YOUR HEALTH! Remember, you are at the center of everything we do.
