An FQHC is a federally funded health center that provides complete primary care and preventative services to medically underserved communities regardless of the patients’ ability to pay.

Anyone can receive care at an FQHC including uninsured, underinsured, Medicaid, Medicare, and privately insured individuals.

FQHC’s provide a wide range of services including:

• Primary care (pediatrics (Children), family medicine, internal medicine (Adult)
• Dental care
• Behavioral health and mental health services
• Chronic disease management (i.e., diabetes, high blood pressure, heart disease)
• Prenatal and women’s health services
• Pharmacy services
• Outreach and health education
• Social supportive services
• Foot care
• Family Planning services (i.e., birth control, safe sex support)
• Routine screenings and vaccines for all ages

No, FQHCs provide services to all individuals, regardless of their insurance status. They offer a sliding fee scale based on income and family size for uninsured patients.

The sliding fee scale is a payment model that adjusts the cost of services based on a patient’s income, family size, and ability to pay.

Each day we offer same day appointments for patients who have urgent care needs. For routine care, it is best to schedule an appointment.

Services are not entirely free, but fees are often reduced based on income. Some services may be covered by grants and other programs for eligible patients.

FQHC’s are not emergency care facilities but can treat minor injuries and illnesses. For life-threatening emergencies, patients should visit an ER or call 911.

• Photo ID
• Proof of income (for sliding fee scale eligibility)
• Insurance cart (if applicable)
• List of current medications
• Medical health records

Yes, FQHCs often provide interpretation services to ensure language is not a barrier to receiving care.
