You’ll be cared for by a dedicated team of doctors, nurses, social workers, community health workers, navigators, and health educators—many of whom are from the very communities we serve and who understand your culture and speak your language. Whether you're caring for a child, a parent, or yourself, our diverse team is here to help your family stay healthy, prevent illness, and find the support you need to live with confidence, connection, and peace of mind.

BCHN's Community Health and Wellness program offered services to 38,255 people in 2024, advancing health equity and strengthening community partnerships across the Bronx.
Approx. 16% (5,639) screened positive for one or more social needs
Nearly half of those had two or more social needs
The top three reported social needs were Housing Quality, Food Security, and Housing Security
760 had housing-related needs
97% received assistance
229 housing applications were successfully submitted
232 had food-related needs
111 were connected to food pantries
86 assisted with SNAP applications
24 assisted with Cash Assistance applications
11 connected to WIC
