Our mission

At BCHN, we are committed to improving the health and well-being of our Bronx community. That means providing access to the highest quality health and wellness care for people of all races, genders, sexual orientations, income levels, and life circumstances.

Through our health care centers, our programs to encourage healthy lifestyles and prevent and manage chronic illnesses, and our outreach to families, teens, adults, and elders, we are creating a more equitable and community-centric model of care for all.

History
With a mission to bring better, more accessible health care to the underserved Bronx, the nonprofit Bronx Community Health Network was founded in 1996.

Just a year after our founding, we received our first federally qualified health center grant and began working closely with Montefiore Health System and Promesa Systems to expand access to care. We soon began offering HIV/AIDS services at ten Montefiore locations and opened the first of our nine school-based health centers—bringing care directly to students where they learn. Today, our network has grown to 18 locations, serving nearly 100,000 patients every year.

In the early 2000s, we introduced oral and mental health services to better care for the whole person. We also launched our Community Health Worker (CHW) program—meeting people where they live and helping them connect with care and vital support services. Over the years, our CHW team has grown into one of the most effective and trusted parts of our care model.

We’ve always believed in supporting families from the very beginning. Through programs like Healthy Family, Healthy Baby, and later Healthy Start—developed in partnership with the Albert Einstein College of Medicine—we’ve helped parents and children thrive by offering the tools, care, and guidance they need for a healthy start in life.
